I, request authorization to participate in LOTUS Fitness online classes and programs. I acknowledge that my participation with LOTUS Fitness is expressly conditioned on my agreement to each of the terms of this document. I acknowledge and agree as follows:

I acknowledge that physical exercise can be strenuous and subject to risk. LOTUS Fitness urges you to obtain a physical examination from a doctor before using any exercise equipment or participating in any exercise activity.

I agree that if I engage in any physical exercise or activity sponsored by LOTUS Fitness, I do so entirely at my own risk.

I agree that I am voluntarily participating in these activities and I assume all risks of injury or illness. This includes, without limitation, my participation in any activity, class, program, or instruction.

I expressly agree to release and discharge LOTUS Fitness, LLC, and all affiliates, employees, agents, representatives, successors, or assigns, from any and all claims or causes of action. This waiver and release of liability include, without limitation, all injuries to you which may occur as a result of (a) your use of any exercise equipment and (b) our instruction or supervision.

By my electronic signature below I acknowledge that I have carefully read this waiver and release and fully understand that it is a release of liability. I agree to voluntarily give up any right that I may otherwise have to bring legal action against LOTUS Fitness LLC. for negligence, or any other personal injury or property damage or loss action.
